Downloading  and completing the paper version of the application will aid you in preparing for the completion of the online version at the above link. Downloading this file will allow you to print a blank hard copy that you can hand write responses on so you will know you have all the necessary information available.  Once you download this file, you will only be able to print it out and then hand write the responses. This downloaded version does not replace the required computer application. The document you download will not allow you to complete the application by computer.

Depending on the size of your program, the completion of the online application could take up to 90 minutes. All applications must be submitted electronically, we will NOT accept paper applications.

Caution: You must be able to complete the online application in one session because the software program does not have the ability to save an unfinished application.

To aid you in the completion of this process, be sure you:

• Know program policies and procedures
• Know how many clients have been served in the last year
• Have the following staff information available for each person who works in the program
• Date(s) of hire
• Degree(s)
• Individual staff training records for last 12 months
